Your plan coverage includes life insurance, dental care, prescription drugs, vision care and more. At hire, you should have completed enrollment forms and after three (3) months of employment, you should receive letters from your Benefit Plan Administrator (Industrial Alliance) that includes a drug card and a username/password for managing your account online. If you need to fill out benefit enrollment forms, please click here. Once completed, you can give the form to your GF, mail it in to the Association, or provide to your LMS Payroll Department.
